What the interest form collects

The form asks for a parent or caregiver’s contact information; a student’s first name, age, and grade; areas of support; academic needs and strengths; preferred format; general availability; and optional descriptions of functional academics, life skills, or parent support needs.

Why this information is collected

Responses are used to understand demand, plan potential services and times, identify which student an inquiry concerns, and follow up with the parent or caregiver.

How form information is handled

Form submissions are sent by email through a third-party delivery provider. The website does not create student accounts or intentionally store submissions in a separate website database.

Information you should not submit

Do not send a student’s last name, birthdate, address, school name, student ID number, diagnosis, medical information, IEP documents, evaluations, school records, therapy records, or other sensitive records through the interest form.

Requesting deletion

A parent or caregiver may reply to a Village of Regulation email and ask for their submission to be deleted.

Policies still in development
